HDD read and write error

today in one of the hdd we had error that it cant able to read and write the data
error log

any issue on this it due to bad sector? or kernel issue

That definitely looks like a bad mounted disk. It's multiple sectors that are not reading at all. It's best to start pulling data from backup.

it possible to get the data from that HDD itself?

Probably but you might get some corrupted files too.
It won't hurt to try though.

Can you post the full but relevant dmesg lines? I just want to rule out other possible issues.

Some days before we had such a problem , the best thing to do is to stop the server and make clone of this HDD to another HDD.But there can be some corrupted files.

Hardware is in the process of dying. PRobably a good idea to replace it / sync the data / restore from backups

The HDD is gone. You should either replace the current drive with a new one and attach the bad drive as secondary OR get a new server and migrate the data off this server immediately.

The harddisk has certainly having a problem. Try to recover the data and move it to another hard disk first.
