could be a bad SATA cable could be a bad hard drive could be a bad montherboard do you have a backup? i would suggest first of all to backup the whole server, before you even try to fix anything ! also, try to probe the S.M.A.R.T. of your hdd.

run SMART utilities and see what it returns.

check your Sata cables, replace them if that did not help ... Replace your hard drive.
