Fill a checkbox based upon onclick number (javascript)

I have a pre-written javascript form like so: the button: When I click this button, it selects a # for my pull down menu. Presently, I have a checkbox on the same page that is unchecked. How can I tell this script to check (fill) the checkbox name "myname" only if the # that was selected is 13?
